invitation may
be withdrawn at any time. Apply
within the school office for information regarding registration and the
completion of registration forms.

First Grade

*No early admissions
will be granted.
FLORIDA
HEALTH CERTIFICATE
Florida law
requires that ALL students have current health forms (Form HRS-680, Part A,
B, or C and HRS-H Form 3040) prior to attending school.
Florida
law states that no student may be admitted until the Cathedral School of St.
Jude is in receipt of these forms.
